Fics like To be Coarse, Rough, Irritating, and Everywhere

Perhaps it was fitting that Vader's death came in a shroud of darkness. Perhaps it was fitting that mere moments later, Anakin was blinded by overwhelming light. Vader dies in his son's arms upon the 2nd Death Star. Anakin reawakens in an unfamiliar body in an - unfortunately - familiar planet. While Geonosis wasn't Tatooine, there was still far too much sand for his liking. Of course, finding himself to be in the body of the Clone Trooper who fell alongside Padme doesn't help matters. Especially when he realises that, while he recognises her, all she sees of him is another face amongst millions of clones. (TLDR: Time-Travelling Vader/Anakin who ends up in the body of that one random clone who fell with Padme back in Ep II).
